Controlling blood loss during myomectomy continues to be a challenge facing reproductive surgeons. Multiple options have been described and promoted over the years to achieve this goal. The most overlooked option is correct technique, where surgeons may dissect in the wrong plane and encounter excessive bleeding. The failure to recognize this can be attributed to how white the innermost layers of myometrium can appear when stretched over the myoma. Thus, the inexperienced surgeon incorrectly assumes that they are in the capsule and begin myoma enucleation when a few more layers remain to get underneath the capsule and in the correct plane.
