Optimizing implantation: are we just scratching the surface?
The capacity to improve pregnancy success after using IVF is an incredibly strong driver to the world of a reproductive endocrinologist. Embryo assessment has led the charge with clear benefits, in certain patient populations (e.g., using preimplantation genetic testing) (1). Despite those improvements our success rates maintain at about 50% (2). Implantation has also been improved with endometrial receptivity testing (3) and potentially an endometrial scratch (ESI), with many areas of active study including the role of uterine microbiome and plasma cell infiltration.
