To suppress or not to suppress? If that is the question, has it been answered?
The administration of prolonged courses of GnRH agonists (GnRH-a) before initiating ovarian stimulation has been shown by multiple investigators to enhance pregnancy rates in endometriosis patients who are undergoing in vitro fertilization (IVF) (1). A Cochrane database analysis of three prospective randomized trials including 163 patients with varying stages of endometriosis who received 3–6 months of prestimulation GnRH-a showed significantly higher clinical pregnancy (odds ratio [OR] 4.28, 95% confidence interval [CI] 2.0–9.15) and live birth (OR 9.1, 95% CI 1.08–78.2) rates compared with women with endometriosis who did not receive prolonged agonist therapy (2).
